What temperature controller do you use and why?
They all essentially do the same thing but what was the reason you chose one over the other?
Would you make a different choice if you had to do it over again?

I have two Ranco ones. One for the kegerator (converted chest freezer) and one for the fermentation fridge. I chose them because I found them here really cheap.

What type of extension cord did you use to connect it or how did you do your own wiring?

I have two of the Rancos. I bought both on ebay fairly cheap and neither has given me any issues. For a smaller fridge, a 14 gauge, 3 conductor cord would do the trick.

I just got a medium duty extension cord in the length I thought would work, cut it, and wired it according to the instructions. Don't go too crazy thick with the wires, because you won't even be able to attach it to the ranco. The terminals inside are pretty small.

I have the Ranco pictured on the top. Compact, easy to use, does the job and ergo-correct for my fermentation room.

I have three Rancos. One on my kegerator I use for fermenting, one on my chest freezer I use for fermenting and lagering, and one on another chest freezer I use for storage. They all work great and the link that Kevin posted has great prices. I also have a ghetto thermostat setup on my Son of Fermentation Chiller, but I don't use it anymore.
